If you become a patient of Rooted In Care, Inc., you will be required to review and agree to our “New Patient Agreements,” and/or other provided paperwork which govern the provider-patient relationship and the provision of clinical services.

This Website is not a secure channel for communicating sensitive health information. Any communication you send to us through non-secure features of this Website is done at your own risk. By using this Website, you agree not to hold Rooted In Care, Inc. liable for any damages arising from or related to your submission of PHI through non-secure channels. If you are a patient, please use our secure Patient Portal or call our office directly to discuss any matters involving your health.
